ProlivaLabs provides professional specimen collection services for employers, organizations, and individuals. This includes DOT and non-DOT urine drug test collections, hair follicle collections, post-accident collections, breath alcohol testing (BAT), DNA specimen collections (ProlivaGenX), and mobile on-site collection services. ProlivaLabs does not perform laboratory analysis or testing. All specimens are securely transferred to certified and accredited third-party laboratories for analysis.

Client Responsibilities

• Provide accurate and complete information when scheduling collections
• Ensure your facility or collection site meets applicable safety standards
• Comply with all applicable federal, state, and local regulations governing specimen collection and testing
• Notify ProlivaLabs of any special requirements or regulatory obligations in advance
• Maintain confidentiality of collection-related documentation as required by law

Limitation of Liability

To the fullest extent permitted by applicable law, ProlivaLabs shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability in connection with any claim arising out of or related to our services shall not exceed the amount paid by you for the specific service giving rise to the claim.
